Dance of all types can involve risk of personal injury. All dancers and participants attending a class are responsible for ensuring that they are physically and mentally fit for classes. They must at all times take care of their own personal safety and the safety of their baby, child and/or children.

I understand it is my responsibility to consult with a physician prior to and regarding my participation in dance class. I represent and warrant that I am physically fit and have no medical condition or previous injury that would prevent my participation in this class. I agree to assume full responsibility for any risks, injuries, or damages which I, or my child/children might incur as a result of participating in dance class. I understand it is my responsibility to securely wrap/strap my child in my chosen wrap/carrier and I assume full responsibility for any risks, injuries, or damage caused when using the wrap/carrier for any class. By signing up to a membership or class online, I knowingly and voluntarily waive any claim I may have against Inclusive Dance Academy, Teleah Lindenberg, or any dance instructor.
